The 21st Century Cures Act requires health care providers to give patients access to all health information in their electronic medical records without delay and without charge. Because of this, you may see your test results before your provider has a chance to review them. Please be assured that your provider will contact you with any abnormal results as soon as possible. Current Patients
If you are a current patient and would like to access the new portal, call 414-454-0600 and ask a team member to send you an activation link. The portal can be used to request an appointment, request a refill, communicate with your provider team, view results, pay your bill online and more. Note: To schedule a procedure (colonoscopy, EGD) please call 414-454-0600 and speak to one of our schedulers.

New Patients
If you are a new patient, please call 414-454-0600 to schedule an appointment. Our team can activate your portal when you scheduled.

This portal is still active to access your health information prior to 2/2/2026.
